Position Summary

A registered nurse specially trained in the Hospice concept to provide for the immediate personal, physical, emotional and social needs of the patient/family unit. Provides Hospice education to new patients and their family/significant others, assess for appropriateness for hospice services and admits hospice appropriate patient to Covenant Care.

Qualifications

A minimum of two (2) years of clinical nursing experience preferred. Experience must be in Hospice or in a medical/surgical setting, a hospital, oncology or geriatric facility. A current RN license in the state of Florida or Alabama as applicable depending on assignment. Reliable transportation, valid state of Florida or Alabama driver's license, as applicable depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required. Must possess current CPR certification.

Main Duties And Responsibilities

Perform initial consultation visit to explain Covenant services and establish appropriateness for hospice.

Conduct initial physical assessment of patient for the purpose of identification of immediate problems and follow-up as necessary.

Obtain medical records, as able, to support admission to hospice.

Document a thorough initial assessment on the current status of the patient/family unit.

Provides information regarding other community and healthcare services available if patient is not appropriate for hospice.

Communicate to patient/family a thorough introduction to Hospice philosophy and services available. Obtain a consent for care and appropriate election statements as required by payer source.

Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services. This applies to determinations and consultations with members of the IDG, including the Attending Physician or Hospice Physician, in all circumstances in which the level of care is to be assigned as short-term inpatient care or continuous care, and includes all actions required in Covenant's policies and procedures regarding the placement or removal of a patient from those categories of care.

Responsible for determining and documenting the level of care status of patients, and in particular, inpatient level of care, to ensure the patient is appropriate and meets the criteria for short-term care needed to manage pain and symptoms in an inpatient facility. This responsibility includes appropriate management and documentation with the patient's change in status either to short-term inpatient care or removed from that status, and all monitoring of the care during the inpatient period. This same requirement applies to management and documentation of patients who are placed on a continuous care level. Strict compliance with all rules and regulations, including Covenant Hospice's policies and procedures, is required.

Utilize new learning experiences to promote professional growth and responsible for updating skills through participation in seminars, workshops, etc.

Competently performs all basic nursing skills.

Carry a minimal case load of patients which will fluctuate between admission and cases, with admissions being the priority.

Complete clinical competency orientation within the 90-day probationary period

Promote a positive image of the Hospice concept throughout the community.

Perform other duties as may be assigned by Care Navigation Manager.

Required to comply with all Covenant Care policies, procedures and personnel policies, including those specifically addressing disaster preparedness and response.

Special Demands/Physical Requirements

This position frequently requires medium/heavy lifting. Normally such lifting will involve, but not be limited to, assisting with the transfer/repositioning of the patient in the home or nursing facility. This position is also mentally demanding because of the continual need of assessing and evaluating clinical aspects of the patient's care. In addition, this position requires considerable time traveling in a motor vehicle in connection with performing essential functions of the position.
