Utilizes leadership skills to coordinate resident care services, serves as resident advocate, facilitates and ensures communication/collaboration among all agency team members.Requirements:The ideal candidate will meet the following requirements:Current state license as Registered Nurse (RN)Minimum of two years' home health experiencePrior knowledge and experience with MedicareStrong clinical assessment and case management skillsTo protect the health and safety of our residents, employees, and the communities we serve, Acts has implemented a universal COVID-19 and Influenza (Flu) vaccination requirement for all employees.
