Job Description:
A company is looking for a Telephonic Social Worker, Care
Manager for remote positions in multiple states.
Key Responsibilities:
Identify and resolve barriers that hinder effective care
Ensure patient is progressing towards desired outcomes by
continuously monitoring patient care
Create member care plans and assess members' social needs
Required Qualifications:
Master's degree in Social Work
Current valid social work license
Minimum 2 years of experience in identifying and accessing
resources to meet basic needs
Must possess effective telephonic and virtual communication
skills
Proficiency with Microsoft Office Programs
